Dear Colleagues,
An updated version (October 2008) of the Proposal Transmittal Form (PTF) is available for immediate use and may be found on the URA website at http://researchadmin.uchicago.edu/about/ptf_1008.pdf
Please note the following significant changes:
| Section I | A new question pertaining to multiple PI applications |
| Section III | A new question regarding Fermilab involvement A new question regarding foreign sponsors or other foreign participation |
| Section V.C. | Cost sharing questions have been expanded |
| Section VI | You may now designate an 'other investigator' as PI for multiple PI applications A new question has been added to verify cross-divisional approval |
| Section VIII | The Oversight Committee Information has been expanded to allow for recording of animal species (choose from a dropdown menu, or add a species not included on the dropdown) A dropdown menu has been added for you to indicate protocol status |
| Section IX | Responsible Administrative Department - you are now asked to designate ‘academic department responsible for this project’ |
| Section X | PI declarations section has been modified in concert with updated COI process - see http://researchadmin.uchicago.edu/regulations/coi.shtml |
For proposals already in the pipeline using the old PTF, we will continue to accept it until November 17.
