Transmittal Forms
- Transmittal to URA (Word)
- Please use this form as a cover sheet for all materials (other than those covered by a PTF) that you are sending to URA
- Concurrence Memo (Word)
- Please include a concurrence memo with any correspondence that you are forwarding to URA.
- Proposal Transmittal Form (PTF) (PDF)
- This form must accompany all proposals routed to URA except--eSNAPs, Clinical Trials, and Material Transfer Agreements (See below for these specific forms.)
- Instructions on how to complete the PTF (Word)
- Faculty/Senior Investigator Effort and Salary Summary Continuation (PDF)
- Oversight Committee Information (Protocols) Continuation (PDF)
- NIH Certification Statement (Word)
- This form is required by NIH and must accompany all NIH proposals.
- eSNAP Transmittal Forms (eTF)
- The eTF replaces the PTF on all eSNAP submissions.
- Clinical Trial Transmittal Form (PDF) (Excel)
- This form must accompany Clinical Trial proposals routed to URA.
- Material Transfer Agreement Routing Form (Excel)
