University of Chicago Individual Conflict of Interest
As of October, 2008, the University Conflict of Interest Assurance and Disclosure forms were revised to reflect the updated University Policy, and to capture University of Chicago Medical Center Conflict of Interest reporting requirements. Process, Contact Information, Policies and Resources are accessible below.
Note: To facilitate compliance with applicable law, reporting obligations and institutional policies, information disclosed on the Assurance or Disclosure forms below may be shared with officials and administrators responsible for conflict of interest oversight at both the University of Chicago Medical Center and the University of Chicago.
Process:

The first step, required of faculty and investigators, is to submit the online Conflict of Interest Assurance Form (COI1). This is done online.

NOTICE: OFF-CAMPUS ACCESS TO COI ONLINE SUBMISSION SYSTEM
Some faculty have reported problems with access to the COI Online Submission System. These may be related to security restrictions in firewalls, especially from off-campus locations. If you experience any difficulties with access please contact Xiomy Rodriguez-Fahs at xrf@uchicago.edu, or (773) 702-2098. We apologize for any inconvenience, and will work with you to address the problem or provide an alternative mechanism for submitting your COI Assurance. -
If, as part of step one, you answer “yes” to any of the six questions, you are required to fill out a Disclosure form (COI2).

The form is an Adobe PDF file. You will need Adobe’s free Acrobat Reader to open and fill it in. If Acrobat Reader is not already installed on your computer you can download it from Adobe’s web site. The completed form should be printed, signed, marked “Confidential”, and sent to Xiomy Rodriguez-Fahs at University Research Administration, 6030 South Ellis Ave, Room 114, Chicago, IL 60637. You can also submit electronically by scanning the signed document and attaching it to an email to coi@lists.uchicago.edu.
