University of Chicago Conflict of Interest Assurance

On May 18, 2001 the Vice President for Research announced an effort to update the University's records of assurance regarding conflicts of interest. If you have been referred to this site to file a Conflict of Interest Assurance of Compliance (also known as COI Form 1), you should find all that you need here.

Process:

  1. The first step, required of all PIs and Key Personnel, is to fill out a Conflict of Interest Assurance Form (COI Form 1). This may be done online (see instructions below) or by downloading and submitting a hard copy (see Downloadable Assurance Form).

  2. If, as part of step one, you are further required to fill out a Disclosure Form (this depends entirely on your answers in the Assurance Form) please see Downloadable Disclosure Form (there is no online system yet for submitting the Disclosure Form).

Online Assurance

To use the online assurance system you access it by clicking Login below. Before you login, you must:

  • Have been issued a personal identification number (PIN) for the system. This should have arrived in an e-mail from the Vice President for Research. If you would like to request a PIN, please email your request to coi-help@ura.uchicago.edu.
  • Have your social security number or your Chicago Card Number (ISO number). This is a sixteen digit number that appears under your name on your University of Chicago ID card.
  • Be visiting this web site from a computer located on the campus network (uchicago.edu) or the hospitals (uchospitals.edu)

When you click Login, a second browser window will open containing the online assurance application. Links to sections of the policy will open here.

Downloadable Assurance Form (COI Form 1)

If you are unable to use the online system (you have not been issued a PIN, are not on campus, or are unable to provide your card number or social security number) you can download an electronic copy of the assurance form (COI Form 1) by clicking COI Form 1 below. The completed form should be submitted to University Research Administration, 3rd Floor University Bookstore, 970 E 58th Street, Chicago, IL 60637.

The form is available as an Adobe PDF file. You will need Adobe's free Acrobat Reader to open and print it. If Acrobat Reader is not already installed on your computer you can download it from Adobe's web site. The form is also available as a Microsoft Word document for those people with Word '97 or higher.

Downloadable Disclosure Form (COI Form 2)

If you have recently filed an assurance (either online, or with COI Form 1) you may have been asked to complete the longer Disclosure of Information Regarding Outside Professional and Commercial Interests/Conflict of Interest form (also known as COI Form 2). You can download a copy of this form by clicking COI Form 2. The disclosure form, like the assurance form, is available in Adobe's PDF format, or as a Word '97 document. There is no online system for completing the disclosure form. The completed form should be submitted to University Research Administration, 3rd Floor University Bookstore, 970 E 58th Street, Chicago, IL 60637.
